The Differences Between TRAM Flap & DIEP Flap Breast Reconstruction
Women who are interested in undergoing breast reconstruction surgery after breast cancer surgery have more options than ever before. And while this abundance of choices is great for ensuring each woman finds the reconstruction plan most beneficial for them, it can be difficult to differentiate the different procedure methods. In addition to reconstruction procedures using breast implants, autologous breast reconstruction using the patient’s own natural tissue can provide fantastic, often life-changing results. Today, our board-certified plastic surgeons wanted to discuss the difference between TRAM flap & DIEP flap breast reconstruction.
The TRAM Flap Method
The TRAM flap breast reconstruction method was actually originally created and implemented by our own practice co-founder, Dr. Carl Hartrampf. This surgery utilizes skin, tissue, and muscle in the lower abdominal area (between the waist and pubic bone) to reconstruct the breast after a breast cancer procedure like a mastectomy. Because this tissue is very similar to the feel and texture of natural breast tissue, it makes for a seamless substitute.
The DIEP Flap Method
Unlike TRAM flap surgery, the DIEP flap procedure does not impact any muscle when rebuilding the breast. During the procedure, incisions are made in the abdomen. Skin, fat, and attached blood vessels are then used to recreate the natural breast contour. The entire process normally takes between 6 and 8 hours. A second procedure (a form of microsurgery) is then needed to attach the tiny blood vessels in the flap to the blood vessels in the new breast.
Choosing between these two options depends on the specific preferences and anatomy of each individual. Both methods can be performed immediately (at the same time as the breast cancer surgery) or at a later date. Some women without enough tissue in the belly area, or whose blood vessels are too small, may not be able to safely undergo a DIEP flap procedure.
